Here’s how you add a contact form using Jetpack for WordPress powered websites:
- In Jetpack, navigate to list all the features and activate the one that says Contact Form.
- Create a new post or page where you want the contact form to go.
- In post compose box, click the button that says Add Contact Form.

Add new fields and remove the ones as per your wish.
